Objective:To assess the safety of sildenafil and dofetilide in patients with pulmonary arterial hypertension (PAH) in a population-based cohort of adult patients with PAH who met the WHO II criteria for PAH.
Patients:The study was a prospective, randomized, double-blind, parallel-group, multicentre, open-label trial with two sites in the USA and 2 sites in the UK. The trial was conducted at the University of Liverpool, United Kingdom. The protocol was approved by the Research Ethics Committee for Human Research of the University of Liverpool, UK (reference no. 09/0.039).
Interventions:Patients were initially assessed for PAH using the International Index of Echocardiography (IIEF) scale and the International Pulmonary Arterial Hypertension (IIHP) questionnaire. The IIEF questionnaire is a comprehensive questionnaire consisting of 5 items: 1) heart rate (HR), systolic blood pressure (SBP), diastolic blood pressure (DBP), and pulmonary vascular resistance (PVR) at time of assessment. In addition, the IIHP questionnaire is a combination of 5 questions: 1) HR, pulmonary artery pressure (PAP), and pulmonary vascular resistance (PVR); 2) SBP, diastolic blood pressure (DBP), and pulmonary vascular resistance (PVR); 3) DBP, pulmonary artery systolic pressure (PASP), and pulmonary artery distensibility (PAD); 4) PVR, pulmonary artery distensibility index (PADI); 5) HR, systolic and diastolic BP, and HR, pulmonary artery distensibility index (PADI-R).
Results:The study group included 70 patients with PAH, which included 29 males and 15 females with age ranging from 18 to 90 years (mean age of 64 years). The median age was 55 years with a range of 18 to 84 years. The mean PAP was 34 mm Hg, the mean DBP was 28 mm Hg, and mean HR was 39 beats per minute (range: 15 to 45 beats per minute). The median PVR was 0.4 mL/m2, the mean PADI was 4.8 mL/m2, and the mean PADI-R was 3.2 mL/m2. There were no significant differences between the groups on any of the demographic and clinical variables.
Conclusions:Although the IIEF questionnaire is highly sensitive and accurate for assessing HR and PAP in patients with PAH, the IIHP questionnaire is a valuable tool for the assessment of HR in patients with PAH.
